Care Manager MSW The Clinical Social Worker collaborates with patients, families, and the healthcare team to address and advocate for the social and emotional needs of patients. Responsibilities include delivering a comprehensive array of social work services, such as psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis management, and resource referrals. The Clinical Social Worker works closely with patients and their support systems, as well as interdisciplinary teams, both within and outside the organization, to enhance patient care through effective resource utilization and monitoring. This role also involves taking a leadership position to help achieve patient goals and desired clinical, financial, and resource outcomes, guided by the NASW Code of Ethics and the organization’s Mission and Vision. Essential Qualifications: Master's Degree in Social Work from an accredited institution. Upon hire: Washington Clinical Independent Social Worker Associate License. Minimum of 1 year of experience in acute medical or mental healthcare settings, either inpatient or outpatient. Salary Range: Minimum: $45.66, Maximum: $69.47 Preferred Qualifications: ACM/CCM certification. Care Manager LCSW The Clinical Social Worker engages with patients, families, and the healthcare team to advocate for the social and emotional needs of patients. Responsibilities mirror those of the MSW role, including psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis management, and resource referrals. The Clinical Social Worker collaborates with patients and their support systems, as well as interdisciplinary teams, to enhance patient care through effective resource management. This position also involves leadership to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes, adhering to the NASW Code of Ethics and the organization’s Mission and Vision.
