Job Description Job Description Job Summary: The Physical Therapist is responsible for planning and delivering physical therapy services to patients in their homes, in accordance with the Plan of Care and agency policies. This role requires specialized knowledge, clinical judgment, and the ability to work independently in a home health setting. Key Responsibilities: Travel to patients’ residences to provide physical therapy services. Conduct OASIS assessments, physical therapy evaluations, diagnostic tests, skilled procedures, and ongoing evaluations under physician supervision. Develop and update individualized care plans based on patient needs and agency policies. Document all assessments, interventions, and outcomes in the electronic medical record in a timely manner. Communicate care plans and updates to physicians, case managers, and other caregivers. Qualifications: Graduate of an accredited Physical Therapy program. Current license to practice as a Physical Therapist in the applicable state. CPR certification required. Home health experience preferred. Job Requirements: Valid driver’s license and auto insurance. Negative TB test and other health screenings as required by agency policy. Ability to perform field work and travel to patient homes.
