Job Overview Start Date: December start - 2 days per week
Setting: Outpatient Clinic Hours : 8am-5pm Patient Volume: 8-10
patients per day Job Details: Perform Disability Benefit
Questionnaires with patients who are Active Duty or Veterans of our
Armed Forces. This provider will be performing Traumatic Brain
Injury (TBI) DBQ’s and submitting their findings and the DBQ form
through the VA portal. EMR: Web based portal used to fill out the
DBQ and submitted to the VA. Delta Locum Tenens can assist with
travel arrangements, credentialing, and will cover malpractice.
