Job Description:
Under the direction of the Wound Care Center Clinical Nurse
Manager/Clinical Coordinator, the Registered Nurse (RN) primarily
provides patient care and handles intake and discharge processes
for the Wound Care Center patients as scheduled. The position may
also serve as a Case Manager to a group of assigned Wound Care
Center patients, as assigned and depending on the Wound Care Center
setup
Provides patient care to Wound Care Center patients and handles
patient intake and discharge processes as assigned and according to
the hospital and nursing standards.Assist in vein clinic as
assigned after Healogics Vein Clinic training completedMay also
serve as Case Manager to a group of patients as assigned.This
entails: Performing the initial nursing assessment, communicating
findings with the assigned physician, assessing the wound stage and
measurements, documenting care plans, evaluating the patient'
status in regards to desired outcomes, and identifying patient care
situations that require interventionParticipating in quality
improvement initiatives, providing education to the patient or
caregiver, and serving as a patient advocate in the delivery and
coordination of the patient care.Performs hyperbaric oxygen therapy
(HBO) related duties as assigned, upon completion of the Healogics
hyperbaric training.Collaborates as needed with other healthcare
providers, Wound Care Center Providers, the Program Director and
the Medical Director regarding clinic and patient needs.May
function as a Documentation Assistant (scribe) in accordance with
Healogics policy. Performs other duties as required.Performs plan
of care intervention, including medication administration, specimen
collection, clinical treatments, as well as other medical care
treatment.Monitors, records, and communicates patient condition as
appropriate to care team, physician, patient, and
family.Collaborates as needed across disciplines to coordinate
patient care, including patient transfer, discharge, referral and
spiritual/psychosocial support needs.Evaluates learning needs of
patient and/or family and provides patient/family education
appropriate to age, culture, condition, and circumstances.

Minimum EducationAssociate's Degree in Nursing X Required
Graduate from an accredited Nursing Program
Required SkillsCertifications:X If employee has ACLS (American
Heart), they do not need BLS.X Wound Care Certification X
Preferred
Requires critical thinking skills, decisive judgment and the
ability to work with minimal supervision. Must be able to work in a
stressful environment and take appropriate action.Must be able to
coordinate the delivery of patient care; completes required
documentation, participates in patient/family teaching and
discharge planning , effectively utilizing the Nursing
process.Provides leadership by working cooperatively and
effectively with physicians, ancillary staff and other
administrative and support personnel in maintaining the standards
of professional nursing practice in the multidisciplinary clinical
setting.Maintains quality delivery of patient care by completing
patient assignments in compliance with the Nursing process and the
Arizona Nurse Practice Act, The Joint Commission, State regulations
and established Hospital Policies and Procedures.Strives daily to
give all customers very good care and promotes Havasu Regional
Medical Centers Standards of Behavior and other duties as
assigned.
Minimum Work Experience2 years clinical nursing required.
Experience in physician office setting, home health or wound care
setting X Preferred.
Required Licenses[Arizona, United States] Registered Nurse
Compact/multistate licensure accepted
